2. Collection and use of personal information

In addition to cookies, we collect personal information necessary to provide our medical and administrative services. This information may include:

  • Identification information: last name, first name, date of birth, health insurance number.

  • Health information: medical history, symptoms, reason for consultation.

This data is collected directly from you (via forms, by phone, or in person) and is used solely for the purposes determined at the time of collection (appointment management, maintenance of medical records, communication with the patient).
